Common use of Future Service Development Clause in Contracts

Future Service Development. The Care Act 2014 sought to put in place significant changes in how health and social care services are delivered, which all authorities and agencies working in this sector need to respond to. The Better Care Fund presents an opportunity to integrate provision of help with home adaptations across housing, health and social care systems to achieve better health and wellbeing outcomes. The success of this regime will be measured by outcomes including:  Numbers of people admitted to residential and care homes;  Effectiveness of reablement;  Delayed transfers of care (‘bed-blocking’);  Avoidable emergency admissions; and  Patient / service user experience The HIA will be well placed to develop enhanced services to contribute towards these outcomes, for example by enabling people to continue to live independently at home rather than being admitted to residential and care homes. The table below uses the workshop results to give a view, based on the mapping exercise, of which activities would be performed by the shared service, and which by council partners.
