Future Direction. First and foremost, a verification study is necessary to compare the Illumina HumanMethylation 450K and Targeted Bisulfite Sequencing methods. Although it is unlikely to have caused errors that would significantly change the findings, any potential differences in their performances will help future decision-making on methodology. Here I propose a study design (Figure 18) that will address the limitations and more importantly, the unanswered questions. Methodologically, since there are clearly defined regions of interest within the genes XXX0, XXXX0, and VMP1 already, the new study should use a single consistent method over its course. Regarding the material, the new study should recruit a greater number of participants to account for possible dropouts. Ideally, the study should have more than 30 patients, of whom half have received chemotherapy, with full methylation profiles over the course of the study. In addition, although it may be difficult to obtain hematopoietic stem cells for study, efforts should still be made to cover, for example, the myeloid and lymphoid progenitors that may pass epigenetic configurations during differentiation, to aid the search for the origin of any methylation changes and to better understand the mechanism of CRF. Research suggests that DNA methylation plays a role in genomic imprinting and is heritable across cell divisions (Xxxxx and Xxxxxxx, 2002; Xxxxxxxxx et al., 2015). It will be important to collect methylation level at CpG sites of interest from blood cells at different stages of differentiation to locate the primary change and trace the possible inheritance of the change in methylation. Furthermore, the new study will set the baseline time point prior to any form of treatment, so that both pre-treatment and post-treatment methylation levels, inflammation levels, and CRF evaluations will be available for comparison. Finally, if the study design above confirms the significance of particular CpG sites, one can search for transcription binding sites in close proximity and determine whether the CpG site methylation has potential impact on cellular functions, which will serve as a step-stone to uncovering the molecular mechanism of CRF.

Future Direction. This thesis work gave an in-depth perspective into the lives of ten YBMSM in Philadelphia. Protective heuristics such as trust, connection with a partner and partner’s physical appearance are some of the individual factors that may affect rates HIV rates in this population. Insufficient knowledge, mistrust in health care advancements, such as PrEP, and lack of health messaging are some of the interpersonal factors identified. Despite having negative or sympathetic thoughts about someone who has HIV, despite high reported testing frequently, despite the intention to not have CAI, this population continues to engage in behaviors that put them at risk for HIV. The perceived low-to-moderate threat of HIV should pique Philadelphia health official’s interest to further investigate this phenomenon. On-going research using qualitative methods should delve deeper into the themes that emerged from this study and address some of the limitations identified. Extending the focus to include psychological and behavioral impact of family support, coping methods, community identity and additional partner heuristics would be essential in expanding knowledge. The findings of this study demonstrate the need to move beyond the stereotypic prevention messages of using condoms to a more dynamic appreciation of the factors that influence risk taking behavior. Furthermore, topics such as masculinity, community identifications and stigma warrant further exploration in the Philadelphia context in both quantitative and qualitative methods. Development of more effective intervention strategies requires a better understanding of factors driving sexual risk taking among YBMSM. While this study found relationship dynamics to deeply influence the quality of the relationship and potentially have a bearing on the agreements individuals make to prevent HIV, there is far more to learn about the contexts in which behaviors take place. While negative thoughts and fear about HIV do not seem to influence self agency in using condoms, talking about HIV status, or other protective heuristics, prevention efforts need to do a better job of addressing stigma and social rejection in this population. Philadelphia needs to expand prevention efforts outside of the traditional sense of clinical settings. Future Direction. Expanding the Geographical Reach of Survey Questions: Questions on disability type and risk of being a victim of violence were developed by a partnership between the researcher, colleagues at the Xxxxxx Center and AIFO, an Italian NGO that provides community-based rehabilitation programs to community members living with physical disabilities and mental illness (29, 37). The data collected within the study served as a pilot of a questionnaire on experience with and perceptions of disabilities, and the questionnaire was developed as a tool to collect standardized data on perceptions of disabilities throughout all of Liberia’s 15 counties in the future. By expanding the geographical reach of these survey questions, a more holistic perspective of community and regional-level health needs can be established. Through this process, improved information on the targeting of relevant and utilizable health clinics can be garnered and outreach strategies can be targeted to meet the needs of individuals living in a multitude of communities. Expanding Research to Include Adolescent Experiences of SGBV: The UN Mission in Liberia (UNMIL) reports that most victims of rape are between the ages of 10 and 19 (6); another recent estimate of SGBV crimes has indicated that over 55% of SGBV survivors in Liberia are less than 14 years old (10). Given this knowledge and the fact that the quantitative survey discussed herein targeted only those over the age of 18 and asked respondents to self-report personal experiences with SGBV, a large proportion of crimes were likely missed and the true proportion of individuals experiencing sexual or physical abuse in each community was very likely underestimated. Future research must target youth at-risk of violence to evaluate their experiences with SGBV and to understand the social and cultural context that victimization occurs within and to assess subsequent health care needs. Expanding Research to Include Experiences of SGBV for Persons Living With Disabilities (PWDs): Additionally, the Liberian government and UNMIL should be encouraged to collect and disseminate data on disabilities as per tenets of the UN’s Convention on the Rights of Persons with Disabilities in order to identify the number of individuals living with disabilities and in order to protect these vulnerable persons (42).
