For Minors the adult accompanying a minor is responsible for full payment. For unaccompanied minors, non- emergency treatment will be denied, unless the responsible parent or guardian has signed a release for treatment, and there is payment by cash or check at time of service or insurance coverage verified. In such case, any applicable coinsurance or copayment must be paid in full.
