Final Summative Evaluation. 1. All formal observations and walkthroughs, will be consolidated into a written summative report using the Evaluation Form in Appendix D within ten (10) working days of the final formal observation and/or walkthrough. A copy will be given to the teacher, and a conference will be held between the teacher and evaluator at the time the summative report is provided. 2. The evaluation shall acknowledge one (1) area of refinement and one (1) area of reinforcement. The evaluator will note all the evidence (if previously discussed with the teacher and substantiated by evidence) used to support the conclusions reached in the formal evaluation report. The evaluation report will be signed by the evaluator. The evaluation report should then be signed by the teacher to verify notification to the teacher that the evaluation will be placed on file, but the teacher’s signature should not be construed as evidence that the teacher agrees with the contents of the evaluation report. Should the report contain improvements needed, they will be specific, measurable and include specific assistance the teacher will be given by an administrator. Costs associated with specific assistance will be paid by the District. 3. The Administration will evaluate each teacher who received a rating of Accomplished on the teacher’s most recent evaluation rating once every three (3) school years. 4. The Administration will evaluate each teacher who received a Skilled on the teacher’s most recent final summative evaluation conducted under this section at least once every two (2) school years. 5. Should a teacher disagree with an evaluation, the teacher may file a written response within thirty (30) working days of receipt which will be attached to the appropriate form. The Teacher Performance Rating used in the Final Summative Rating shall be calculated in the following manner. 1. The Administration will evaluate each teacher who received a Skilled on the teacher’s most recent final summative evaluation conducted under this section all criteria being evaluated at least once every two (2) school yearsdays prior to a final summative evaluation conference, which shall be held no later than May 15th. When possible, the conference shall be held in the teacher’s classroom to facilitate the documentation of artifacts and evidence. The purpose of the summative rating conference is to discuss the following:
i. The evaluator’s preliminary rating on all criteria;
ii. Student growth data; and
iii. Any additional evidence and artifacts the teacher would like the evaluator to consider before making a final summative rating.
5b. Nothing prohibits an evaluator from evaluating all teachers as Distinguished-4 within a school year.
c. No criterion shall be marked unsatisfactory or basic on the overall rating without first being brought to the employee’s attention.
d. With the mutual agreement of the teacher and evaluator, this final summative conference may be consolidated with a post observation conference following the final required observation.
e. All evidence, measures and observations used in developing the final summative evaluation score must be a product of the school year in which the evaluation is conducted, except as provided in subsection 8E, in the case of probation extended into the following school year. 5. Should a teacher disagree with an evaluationEach evaluation report shall be promptly forwarded to the employee's personnel file. The distribution of the evaluation form shall be as follows: original to be filed in the personnel file of the employee, the teacher may file a written response within thirty (30) working days of receipt which will be attached first copy to the appropriate formprincipal, and the second copy to the employee.

The Teacher Performance Rating used in If an employee is transferred to another position not under the Final Summative Rating supervisor's jurisdiction, an evaluation shall be calculated in made at the following mannertime of such transfer, provided however, that only one evaluation shall be completed annually.
