Evaluation Form. i. Probationary unit members and permanent unit members shall be evaluated on the standard classified evaluation form. A copy of the evaluations form is attached to the Agreement. ii. All unit members shall receive a copy of the completed and signed evaluation form. In addition, the evaluator and the unit member shall establish goals for the unit member and help the unit member identify areas for improvement, growth, and professional development. The evaluator shall identify support available to help the unit member meet the goals.
