Common use of Dental/Vision Clause in Contracts

Dental/Vision. ‌ 1. The Board shall provide and pay a percentage of the premiums for a single group dental plan, which provides one hundred percent (100%) aid to preventive and diagnostic dentistry, eighty percent (80%) basic restorative service, sixty percent (60%) major restorative services, $1,000 per person orthodontic sixty percent (60%) deductible, $1,500 calendar year maximum per person, $25 individual deductible, $50 family deductible. The plan is provided by ACSCOG. An employee enrolled in the family dental plan shall pay the difference between the single and family premium; however, no employee will be required to enroll in the family plan. Enrollment rights under the Dental Plan shall be subject to the rules of the Cafeteria Plan. 2. The Board shall provide a vision care plan see Appendix.
