Default Pricing. In the event the Terms and Conditions specify that "Default Pricing" shall apply, such work will be invoiced to Owner on a (i) "Time and Materials" Pricing basis, based on the hourly rates of the applicable Contractor personnel, set forth in Attachment 1.2
Default Pricing. For unlisted and/or codes which do not have Medicare fees for a Covered Service, the Maximum Allowable fee will be determined by Regence.
