Credit Application. The Customer authorises Supplier, its authorised agents or representatives, to make enquiries as to the credit and financial history and responsibilities of the Customer, and/or its Representative (as applicable), as required by Supplier from time to time. If Supplier so requires, the Customer must provide (and must procure that its Representatives or guarantors, as applicable, provide) any consent or authorisation required by law for Supplier, its Representatives to undertake any such enquiries, including obtaining reports from credit reporting bodies as to the consumer and/or commercial creditworthiness of the Customer and/or its Representatives or guarantors (as applicable). Any credit terms granted to the Customer is on the condition that Customer has made complete and accurate disclosure to Supplier in its credit application of all material information relevant to Supplier’s decision to extend credit. The Customer must promptly inform Supplier of any material adverse change in Supplier’s financial or business circumstances.
Credit Application. Any new customer must obtain our credit approval before we will arrange for transportation. The following information must be submitted to our Credit Department:
Credit Application. To apply on the salary schedule, all credits beyond the bachelor's degree must be graduate credits and must be in addition to those credits which are necessary for initial certification. All credits will be on a quarter credit basis (i.e., 1.5 x semester credit). Also, credits used toward advancement on the salary schedule must also be earned by completing courses relating to the major teaching field, or as a part of a definite career plan in education, which has been previously approved by the Executive Director of Administrative Services. A portion of acceptable credits may be credits earned from experience designated as "in-service" by the Executive Director of Administrative Services and/or a committee appointed by the Executive Director of Administrative Services for that purpose. Undergraduate credits or in-service credits taken outside the School District may be used toward advancement on the salary schedule upon the prior approval of the Executive Director of Administrative Services, whose decision is final and binding and shall not be subject to the grievance procedure. Three (3) of the fifteen (15) credits for any given lane change may be outside the teacher's major teaching field if related to the field of education with the approval of the Executive Director of Administrative Services, whose decision is final and binding and not subject to the grievance procedure. Credits for any given lane change which are not graduate credits in the major teaching field must be pre-approved by the Executive Director of Administrative Services as part of an established career plan. Such an established career plan shall be submitted in writing to the Executive Director of Administrative Services prior to taking the courses or in-service and shall be subject to the approval of the Executive Director of Administrative Services whose decision is final and binding and not subject to the grievance procedure. New areas of licensure (District required): If the District requires that a teacher take additional coursework (graduate or undergraduate) in order to obtain a new area of licensure (e.g., 5-8 middle level licensure), the teacher will have the option of receiving future salary advancement or receiving reimbursement from the District for the actual cost of tuition and fees related to such course work.
Credit Application. Process. Customer must complete two steps in order to qualify for an SLA credit. First, Customer must open a Trouble Ticket in response to Service issues. This first step brings the problem to the attention of Verizon customer service for intervention and repair, as required. The second step is to request the credit in writing from Customer’s account team contact. The timing of the written request varies by Service Level Standard and is described below. The account team receiving the SLA credit request will confirm receipt with Customer by either email or fax. Verizon will then investigate the outage through the Trouble Ticket history and notify Customer of the outcome of the investigation either by email or fax.
Credit Application. In order to receive college credit(s) toward lane change or district reimbursement subject to article VI Subd. 6. The course(s) must either be related to a teacher’s field(s) licensure or employment assignment. The teacher must obtain prior written approval from the Superintendent.

Large publically traded clients with a D&B 4A rating or better will not require credit applications. The Convergys salesperson or the Lead Desk Support will send the credit application to the client for completion.
