Course Request Process. A. New Courses: To initiate a request to offer a new Type A, B, or D Course, the District Liaison must complete and submit to the College Liaison a Course Request Form for each proposed course. Unless otherwise agreed to by the College, for a Fall semester course, the Course Request Form should be submitted to the College Liaison by no later than February 1 of the same calendar year. Unless otherwise agreed to by the College, for a Spring or Summer semester course, the Course Request Form should be submitted to the College Liaison by no later than September 1 of the calendar year prior to the semester. Exceptions may be made after the deadline if agreed upon by both Parties.

  • Request Procedure The employee shall furnish evidence to the immediate supervisor that leave taken in accordance with the provisions of this section is in connection with family illness. The employee shall notify the immediate supervisor of any of the circumstances necessitating the leave change.
