Core Characteristics. The PMH is the foundation and corner stone of the integrated system of person centered primary and community care as the practice model for delivering key services associated with a full service primary care practice. PMHs are premised on five core characteristics: • Accessible to the patient as therapeutic partner. • Engaged and motivated to achieve health service goals. • Possess the knowledge, skills and competencies to deliver the services. • Work in a safe and healthy environment. • Receive support and leadership.
