Communication & Reporting. 1. You shall communicate to IACA through the designated email addresses, all changes of data and circumstances that may be relevant for your participation in the professional training.
2. Any costs or adverse consequences caused by changed or unknown circumstances on your data, not communicated to IACA are not attributable to and shall not be borne by IACA.
Communication & Reporting. 1. Provide information and consultation to the COUNTY regarding the child’s needs as identified in the emergency shelter, and the care provided to the child in the emergency shelter; and provide information to the COUNTY Placement Unit Supervisor and/or the Placement Unit social worker to assist in the completion of a placement evaluation for each child;
2. Communicate with the COUNTY Placement Unit supervisor and/or Placement Unit social worker immediately whenever a child appears to have an emotional or medical problem which cannot be managed suitably at the shelter, which requires external intervention, or which appears to warrant further action during the child’s next placement;
3. Provide the COUNTY with written incident reports whenever an emergency shelter child is;
a. injured,
b. involved in a crime,
c. Engages in aggression, self-harm, sexual misconduct, property destruction, running away, defiance or provocation.
4. Provide the COUNTY with written incident reports whenever an emergency shelter staff person appears to have acted inappropriately with a child placed in the emergency shelter;
5. Provide the COUNTY with the following routine written reports:
a. The Shelter Assessment Report, within 14 days of placement,
b. The Discharge Progress Report when a child has been in the shelter for two or more weeks beyond the end date of the Shelter Assessment Report, and further emotional, behavioral or medical developments have occurred;
Communication & Reporting. The Local Coordinator will communicate regularly with the FHFH staff by phone, email or in person. Notes from Local FHFH Team meetings and a basic annual report of activities and results will be forwarded to the FHFH main office.
Communication & Reporting. The Chapter Director will communicate regularly with the ULT staff by phone, email or in person. Notes from Chapter ULT Team meetings and a basic annual report of activities and results will be forwarded to the ULT main office at least quarterly, or more often, as needed.
Communication & Reporting. Contractor shall endeavor to keep Owner informed regarding Contractor’s progress to the extent required so Owner can have meaningful review and oversight in Contractor’s efforts. To that end, Contractor shall provide Owner monthly progress reports commencing on the Effective Date, containing the following information: (a) a detailed description of the progress of the Work during the reporting month, including an up-to-date Schedule depicting all critical path activities and illustrating the progress that has been made against such activities; (b) a summary of progress towards obtaining Contractor Permits; (c) a listing of any violations of Permits or applicable Law and actions taken or to be taken to eliminate any subsequent violations; (d) a statement of any significant issues that remain unresolved and Contractor’s plan for resolving such issues; (e) Schedule updates; and (f) any other information reasonably requested by Owner, all in such detail as may reasonably be requested by Owner. Without limiting the generality of the foregoing sentences, Contractor shall, as a matter of course, promptly provide Owner with copies of designs, documents, and any other information related to the Facility as required for Owner’s review and input. If requested by Owner, Contractor shall create, for Owner’s approval, and implement a plan for orderly communication between the parties related to the Facility.

Communication & Reporting a. The City shall report to the Social Services Committee on no less than a quarterly basis, and such reporting will minimally include the following:
i. Comparing actual progress with scheduled progress for the design and construction phases of the project;
ii. Indicate actual or estimated costs compared to the construction budget; and,
iii. Such other information which the City deems appropriate.
b. Any matter requiring approval under this Agreement, is not approved until it is approved by both the City and County Councils.
Communication & Reporting. Supplier shall continuously update Supplier’s NA Stores Help Desk troubleshooting documentation and FAQs regarding the Services based on analysis of issues (“Call Drivers”) that generate a larger than average number of calls or an increased number of calls from the Covered Locations to the NA Stores Help Desk. * Certain information on this page has been omitted and filed separately with the Commission. Confidential treatment has been requested with respect to the omitted portions. Exhibit A.3.2 Gap/Supplier Confidential and Proprietary Information Supplier shall facilitate the meetings and provide Gap with the documents described in Exhibit D.13 (Reports).
