Common use of CO-PAYMENT Clause in Contracts

CO-PAYMENT. You are required to collect the assessedparent fee/co-payment from the parent. a. The amount of the co-payment is listed in the child’s Portchaildlcaree cnerrtifoicaltel. Ifmthee ncot- / payment changes, we will notify you in writing . Only co-payment changes from ELCFH are valid. b. Co-payments are automatically deducted from your monthly reimbursement. c. You must give the parent a receipt for each co-payment collected and retain the receipt records. Upon request, you must provide a current accounting and copy of co -payment receipt records to ELCFH.
