CLIENT CASE RECORDS. 6.1 CONTRACTOR shall maintain all SSA CLIENT records in a single Orange County location. 6.2 CONTRACTOR shall maintain CLIENT records that include, but not be limited to: 6.2.1 Referral from ADMINISTRATOR and any referral documentation provided by COUNTY. 6.2.2 Name, address, and phone number of CLIENT(S) and VISITOR(S). 6.2.3 Birth date, age, and gender of CLIENT.
