Claim Form Availability. The Claim Form shall be in a substantially similar form to that attached as Exhibit 3. The Claim Form will be: (i) included on the Settlement Website to be designed and administered by the Settlement Administrator; (ii) made readily available from the Settlement Administrator, including by request from the Settlement Administrator by mail, email, or calling a toll-free number provided by the Settlement Administrator; and (iii) made readily available via a website address included in the Short Form Notice that will be mailed and/or emailed to members of the Settlement Class for whom Wyndham Hotels has a mailing address and/or email address.

Claim Form Availability. The Claim Form will be part of the Long Form Class Notice and will be available for downloading from the Settlement Website. The Claim Form may also be requested by calling the toll-free number provided by the Settlement Administrator or by writing to the Settlement Administrator. The Claim Form will also exist in an electronic form that can be submitted on the Settlement Website.

At Class Counsel’s option, Class Counsel may establish on their firm websites a link to the Settlement Administrator’s website.
