CIRCUMSTANCES WHEN SHARED CARE IS APPROPRIATE. Prescribing responsibility will only be transferred when the consultant and the GP are in agreement that the patient’s condition is stable or predictable. Patients will only be referred to the GP once the GP has agreed in each individual case and the hospital will continue to provide prescriptions until successful transfer of responsibilities as outlined below. Patients should be able to decline shared care if after due consideration of the available option, they decide it is not in their best interest. The hospital will provide the patient with a minimum initial supply of 4 weeks therapy.
CIRCUMSTANCES WHEN SHARED CARE IS APPROPRIATE. Prescribing responsibility will only be transferred when the consultant and the GP are in agreement that the patient’s condition is clinically and biochemically stable or predictable e.g. no evidence of hypocalcaemia or hypocalcaemia is controlled. Patients will only be referred to the GP once the GP has agreed in each individual case and the hospital will continue to provide prescriptions until successful transfer of responsibilities as outlined below. Patients will have received a minimum of six months treatment with IV bisphosphonate. The patient will have been assessed as being stable and suitable for oral treatment and will have received at least four weeks oral treatment from the hospital specialist.
CIRCUMSTANCES WHEN SHARED CARE IS APPROPRIATE.  Prescribing responsibility will only be transferred when the Child and Adolescent Mental Health Service (CAMHS) and the GP are in agreement that the patient’s condition is stable or predictable and in accordance with NICE guidance.  The patients will only be referred back to the GP once the GP has agreed in each individual case and the hospital will continue to provide prescriptions until successful transfer of responsibilities as outlined below  The hospital will provide the patient with a minimum initial supply of 4 weeks of medication

In the event that the Project Scope is for right-of-way only, notwithstanding Appendix D, the OPWC shall pay for 100% of the right-of-way costs not to exceed the total financial assistance provided in Appendix C.   • Foreign-Owned Companies in Connection with Critical Infrastructure If Texas Government Code, Section 2274.0102(a)(1) (relating to prohibition on contracts with certain foreign-owned companies in connection with critical infrastructure) is applicable to this Contract, pursuant to Government Code Section 2274.0102, Contractor certifies that neither it nor its parent company, nor any affiliate of Contractor or its parent company, is: (1) majority owned or controlled by citizens or governmental entities of China, Iran, North Korea, Russia, or any other country designated by the Governor under Government Code Section 2274.0103, or (2) headquartered in any of those countries.
