Choice of Provider Clause Samples
The Choice of Provider clause establishes which party has the authority to select a service provider or vendor for certain goods or services under the agreement. Typically, this clause specifies whether the client, customer, or another party can choose the provider, and may outline any criteria or approval processes required for the selection. By clearly designating decision-making power, the clause helps prevent disputes over provider selection and ensures that both parties understand who is responsible for engaging third-party services.
Choice of Provider. The Contractor shall provide a beneficiary’s choice of the person providing services to the extent possible and appropriate consistent with Cal. Code Regs., tit. 9, §1830.225 and 42 Code of Federal Regulations part 438.3(l).
Choice of Provider. The CHC-MCO must provide Participants with choice of Providers within its Network. The CHC-MCO may not attempt to steer Participants to Affiliates who are Providers or interfere with the Participants’ choice of Network Providers. Participants may choose a Provider from within the Network at any time, even during a continuity of care period.

Choice of Provider. The resident has the right to choose the licensed, certified or registered provider of health care and pharmacist of the resident’s choice. In addition, a resident has the right to select a Medicare Part D plan of his or her choosing.
Choice of Provider. The selection of a service provider or facility is to be made by the Plan Member and at the sole discretion of the Plan Member. CNN assumes no responsibility of any nature whatsoever for any acts or omissions of any service provider or facility under the CNN Recommendation.

The Provider will assure that the participant is receiving services under this Agreement by the provider of his or her choice, and that referrals to other providers of service will not interfere with a participant’s freedom of choice in seeking medical care from any institution, agency, pharmacy or person who is qualified to perform a required service. If a participant is under 18 years of age or mentally incapable of choice of provider, the Provider will assure that the participant’s legally authorized representative makes such choices for the participant, unless the participant is authorized to make this choice under Federal or State law.
Choice of Provider. Selection of the providers of the Utilities will be carried out jointly by both LCCCA and PSP. If the parties are unable to agree the selection of the provider, this Agreement will be terminated.
