CHANGING YOUR REFERRED SPECIALIST. The Member may change the Referred Specialist to whom the Member has been Referred by a Primary Care Physician or for whom the Member has a Standing Referral. To do so, the Member should ask the Primary Care Physician to recommend another Referred Specialist before services are performed. Or, the Member may call Keystone’s Customer Service Department at the telephone number shown on the ID Card. Only services authorized on the Referral form will be covered. PROVIDER DIRECTORY A Provider Directory is made available to Members via our website at xxxxxxxxxxxx.xxx. A printed provider directory is available upon request. It includes a listing of Hospitals and Primary Care Physicians and Referred Specialists by location, telephone numbers and Hospital affiliation. The Directory also will indicate whether the Physician is accepting new patients.
