Common use of Case Management Clause in Contracts

Case Management. CONTRACTOR shall provide Case Management services by 12 contacting outside agencies and making referrals for services outside the scope of comprehensive 13 substance abuse services as identified in the Client’s recovery. Such concomitant services include 14 academic education, vocational training, medical and dental treatment, pre and post counseling and 15 testing for infectious diseases, legal assistance, job-search assistance, financial assistance, childcare, and 16 self-help programs such as twelve (12) – step programs. Said linkages, referrals and follow-up are to be 17 documented in the Client file.

Case Management. CONTRACTOR shall provide Case Management case management services by 12 35 contacting outside agencies and making referrals for services outside the scope of comprehensive 13 36 substance abuse services as identified in the ClientParticipant’s treatment plan as necessary to the Participant’s 37 recovery. Such concomitant services include 14 academic education, vocational training, medical and 1 dental treatment, pre pre-and post counseling and 15 testing for infectious diseases, legal assistance, job-job search 2 assistance, financial assistance, childcare, and 16 self-help programs such as twelve (12) – step programs. Said linkages, referrals and follow-up are to be 17 documented in the Client file.
