Common use of Authorizing Physician Clause in Contracts

Authorizing Physician. Pharmacy must notify Authorizing Physician within twenty-four (24) hours of administering a vaccine under this Agreement. This notification must include the name and address of patient, age if under fourteen (14) years old, name of primary care physician if provided, date vaccine given, amount administered, manufacturer, lot number, expiration date, site of vaccination and route of administration, and the name, address and title of the person administering vaccine.12
