Arbitration Form. Each notice of arbitration must be submitted in writing on the form shown in Appendix “D” (Notice of Arbitration). All pertinent information submitted with the Grievance Form, as well as the Grievance Hearing decision, must be included as an attachment to the Notice of Arbitration form. The Notice of Arbitration form must be signed by the grievant and a UFF representative.
