Application under clause 34. 3(iv) (i) must be received by the employer during the period of illness or injury. (ii) must be accompanied by a medical certificate from a registered medical practitioner or a statutory declaration attesting to the illness or injury and the duration of the illness or injury, and (iii) must indicate whether the teacher wishes to extend the LSL by the period of the illness or injury or whether the teacher will return from LSL as planned with the period of illness or injury increasing the teacher’s accrued LSL entitlement.
