Applicant Responsibilities. If the certification review indicates missing or non-compliant items, you agree to respond to such exceptions within sixty (60) days from the date notice is given. If the Application remains incomplete or non-compliant sixty (60) days after notification, the Application will be denied. A resubmittal must be made under a new Application.
Applicant Responsibilities. ● The applicant organization will ensure that only the spaces specifically reserved will be used, and the times designated will be observed as indicated on the application. ● Comprehensive adult supervision of the facilities will be provided at all times on site by the applicant organization. At no time will building custodians be responsible for the supervision of participants before, during or after the event. ● The applicant organization shall be held financially responsible for any and all property loss or damage to the facility in any way. (See deposit requirements.) Appropriate school department personnel will determine the cost of repairs and/or replacement. ● Event reservations will be considered tentative until confirmed in writing by the school department’s designee. The school department reserves the right to cancel any event reservation in its sole discretion. ● Any applicant organization wishing to preview, measure, or otherwise view the facility must have permission from the school department’s supervisory designee. ● Applicant organizations must have fully paid for all event costs before future considerations are given or reservations accepted. ● Applicant organizations will be responsible for arranging and paying for appropriate safety coverage – including, but not limited to, police and fire department personnel- and provide proof to the school department supervisor that such arrangements have been made. In addition, the applicant organization must secure an amusement/entertainment license from the City of Methuen as required by city ordinance. ● Applicant organization will provide evidence of appropriate liability coverage for all performers and audience members. A Certificate of Insurance naming the school department as Additional Insured shall be provided to the facilities supervisor. The applicant organization agrees to indemnify and hold harmless the school department for any and all suits, causes of action, damages (including attorney’s fees) and litigation costs arising from any cause of action related to the applicant organization’s use of the property.
Applicant Responsibilities. The Applicant shall: Provide Company with preliminary engineering plans and preliminary plat of subdivision before Company commences any engineering design. Install curbs (when necessary) and provides grades to within 4" of final grade where the gas facilities are to be installed. If curbs are not required, and additional Hold Harmless Agreement shall be signed by the Applicant. Install sleeves at all road crossings and at other locations as required by the Company in the Company's sole discretion, and the end of the sleeves shall be clearly marked. If the sleeves are not installed or cannot be utilized, the Applicant may be billed for additional costs incurred by the Company. Stake and clearly xxxx underground facilities that do not fall under XXXXX. The Company is not responsible for any damage to the unmarked underground facilities and reserves the right not to install gas facilities until such a time as such underground faciltities are marked. Remove all obstructions from the route and area where the gas facilities are to be installed. Stake road right-of-ways, public utility easements and easements to the Company as required. The Company will schedule the installation of gas facilities after the Applicant’s Premises is prepared and, if required by the Company in its sole discretion, the Company may inspect the Premises to determine site readiness for installation of gas facilities. The Company will not be liable for any delays in the installation of gas facilities. No work will be scheduled by Company until the Applicant’s responsibilities are met. If Applicant requires that installation be completed under the following types of conditions, Applicant may be billed for any additional costs incurred by the Company: For installation during the winter construction season, as provided in the Company’s tariff. If Applicant requests directional boring and the Company is able to perform directional boring to install gas facilities, the Applicant shall pay the Company the cost of said boring activities. Applicant Responsibilities. 1. DVDs must not have anything else recorded on them before or after the program. One title and program starts at 1st chapter. Total run time (TRT) must be printed clearly on the DVD along with the title and producer name.
