Administrative Offset. A. If an employee is covered with a family medical plan, but is only eligible for a single plan and has not provided proper documentation to the Cuyahoga DD or its vendor within 30 days of the loss of the dependent, the employee will be responsible for repayment of the excess costs incurred by Cuyahoga DD. The offset may be accomplished by salary reduction, if the employee chooses this option. Acceptable documentation would be official court paperwork, preferably the decree for divorce or annulment, first and last page with received filing date. If notification and documentation is received after 30 days the insurance carriers have the option of reversing out claims that have been paid after the qualifying event. B. The Cuyahoga DD will have a positive reenrollment twice during the term of this contract. Employees will be required to provide proof of dependent eligibility.
