Additional Services Pricing. Fitness For Duty (FFD): $350.00 Fitness For Duty (FFD) is available to leadership within an organization and is indicated when there is an employee exhibiting unusual, bizarre or unsafe behaviors, that pose a threat the themselves or to others. The Fitness For Duty referral has 4 defines phases: 1) comprehensive assessment by an EAP Clinician, 2) treatment/education compliance monitoring, 3. medical clearance 4. Individualized Return To Work Plan. The EAP Clinician will remain available to consult with leadership throughout the Fitness For Duty process. Substance Abuse Services: $800.00 The Department of Transportation (DOT) regulation 49 CFR Part 40, stipulates that employees who work in safety sensitive positions and have violated a drug and/or alcohol regulation, may require services from a Substance Abuse Professional (SAP). The SAP process includes an initial evaluation, treatment/education recommendations, monitoring and follow-up testing. Penn Medicine Princeton EAP has Substance Abuse Professionals on staff and offers SAP services to all organizations. On-Site Management Consultation: $250.00/Hour An on-site meeting between an EAP Specialist and managers of your organizations to improve their performance, through the analysis of existing organizational problems and the development of plans for improvement. Additional Services Pricing. (where applicable) for either Lot shall be chargeable in addition to Mandatory Services pricing. Additional Services pricing will be determined at the Call Off stage in accordance with the Call Off procedure as set out in Schedule 5 of the Framework Agreement.

  • Additional Services Registry Operator shall be entitled to provide the Registry Services described in clauses (a) and (b) of the first paragraph of Section 2.1 in the Specification 6 attached hereto (“Specification 6”) and such other Registry Services set forth on Exhibit A (collectively, the “Approved Services”). If Registry Operator desires to provide any Registry Service that is not an Approved Service or is a material modification to an Approved Service (each, an “Additional Service”), Registry Operator shall submit a request for approval of such Additional Service pursuant to the Registry Services Evaluation Policy at xxxx://xxx.xxxxx.xxx/en/registries/rsep/rsep.html, as such policy may be amended from time to time in accordance with the bylaws of ICANN (as amended from time to time, the “ICANN Bylaws”) applicable to Consensus Policies (the “RSEP”). Registry Operator may offer Additional Services only with the written approval of ICANN, and, upon any such approval, such Additional Services shall be deemed Registry Services under this Agreement. In its reasonable discretion, ICANN may require an amendment to this Agreement reflecting the provision of any Additional Service which is approved pursuant to the RSEP, which amendment shall be in a form reasonably acceptable to the parties.

  • Transitional Services Contractor shall provide Transitional Services to offenders who are being released from a prison, an assessment/sanction center, prerelease center, or treatment center for up to the first 90 days of community supervision after release. A Probation and Parole Officer (PO) will determine the specific services to be provided to each offender based on the offender’s needs and individualized case plan as determined or developed by the State. The referring PO will complete an agreed upon referral form designating the services selected. Contractor will confirm availability of services, establish a start date, and return the referral form to the PO and the designated State staff.

  • TAX SUPPORT SERVICES BNY Mellon shall provide the following tax support services for each Fund:  Provide various data and reports as agreed upon in the SLDs to support TRP’s tax reporting and tax filing obligations, including: · Wash sales reporting; · QDI reporting; · DRD reporting; · PFIC analysis; · Straddle analysis; · Paydown adjustments; · Equalization debit adjustments · Tax compliance under §851, §817(h); · Foreign bond sale analysis (§988); · Troubled debt analysis; · Estimation of income for excise tax purposes; · Swap analysis; · Inflation adjustments; · §1256 adjustments; · Market discount analysis; OID adjustments; · CPDI analysis; · Shareholder tax reporting information (e.g. FTC, UGG income, foreign source income by country, exempt income by state);  Provide data, and reports based on such data, maintained by BNY Mellon on its fund accounting platform as reasonably requested by TRP to support TRP’s obligations to comply with requests from tax authorities and TRP’s tax reporting and tax filing obligations.  Assist with other tax-related data needs as mutually agreed upon in writing from time-to-time.

  • Support Services HP’s support services will be described in the applicable Supporting Material, which will cover the description of HP’s offering, eligibility requirements, service limitations and Customer responsibilities, as well as the Customer systems supported.

  • Optional Services To the extent that the Fund elects to engage the Transfer Agent to provide the services listed below the Fund shall engage the Transfer Agent to provide such services upon terms and fees to be agreed upon by the parties:
