Account Data. Group shall provide to BlueLincs HMO all identifying information for Group and/or Group Participating Provider, including name, address, phone number, office hours, and tax identification number. At least sixty (60) days' advance notice to BlueLincs HMO is required for changes in account data of Group and/or Group Participating Provider.
