Absence Request Form. Employees must fully complete and timely return to the District its standard Absence Request Form to request time off for any of the following types of absence: Vacation Leave Floating Holiday Leave Compensatory Time Leave Sick Leave Pregnancy Leave Jury/Witness Leave Bereavement Leave Unpaid Leaves Employees must complete the Absence Request Form as completely as possible, and sign it. In the case of sick leave, the employee must indicate if the absence is for a doctor’s appointment, dentist appointment, or an illness; and whether it is for the employee, or an immediate family member and the family member’s relationship to the employee. Absence Request Forms are to be submitted by the employee to the employee’s supervisor at least (1) one week before the requested leave, except in cases of sick, leave or emergency. In cases of sick leave due to illness, the employee shall submit to the employee’s (non-unit) supervisor the completed Absence Request Form before he or she may resume work. In emergencies, the completed form must be submitted at the earliest possible opportunity.
