Wellness Vendor definition

Wellness Vendor means an entity with which DCH has contracted to provide wellness and prevention programs.

Examples of Wellness Vendor in a sentence

  • From time to time as requested by the Office of Student Health and Wellness Vendor shall provide additional services to students, CPS Employees, qualified CPS vendors, patient's parent/guardian/patient and/or the community in general including but not limited to Covid-19 vaccination services and any other emergency health related services or safety measures recommended by Center for Disease Control, Illinois Department of Public Health, or Chicago Department of Public Health, etc and as set forth below.

  • Tobacco Cessation/Weight Management/Disease Management/Maternity Management – (Optional Services) The UM Vendor may be responsible for transmitting participant engagement to the Health and Wellness Vendor.

  • From time to time as approved by the Office of Student Health and Wellness Vendor may actively engage in two-way communication to build trust with populations served and ensure accuracy and effectiveness of prevention and health promotion strategies, ensuring public health communications and education efforts are asset based when appropriate and do not reinforce narratives that are damaging to disproportionately affected populations.

  • From time to time as approved by the Office of Student Health and Wellness Vendor may make reasonable outreach efforts to develop and disseminate accessible health information/resources, and to educate patient’s parent/guardian/patient about health including factors that influence personal health, and how to improve personal health.

  • As part of the Services, at no additional cost to Client or Client Employees, Client authorizes Paychex to provide Employees with direct access to the Financial Wellness Service (“Service") provided by the Financial Wellness Vendor (“Vendor”).
