From time to time as requested by the Office of Student Health and Wellness Vendor shall provide additional services to students, CPS Employees, qualified CPS vendors, patient's parent/guardian/patient and/or the community in general including but not limited to Covid-19 vaccination services and any other emergency health related services or safety measures recommended by Center for Disease Control, Illinois Department of Public Health, or Chicago Department of Public Health, etc and as set forth below.
Tobacco Cessation/Weight Management/Disease Management/Maternity Management – (Optional Services) The UM Vendor may be responsible for transmitting participant engagement to the Health and Wellness Vendor.

As part of the Services, at no additional cost to Client or Client Employees, Client authorizes Paychex to provide Employees with direct access to the Financial Wellness Service (“Service") provided by the Financial Wellness Vendor (“Vendor”).
