Examples of VPK provider in a sentence

  • If PROVIDER is executing a single Contract on behalf of multiple private VPK provider sites within COALITION’s service area, a list of their names and their physical addresses are included in Exhibit 1: Provider Location List.

  • PROVIDER’s EIN or SSN will be used for processing payments to PROVIDER as a VPK provider, for reporting those payments for federal tax purposes, and for routine identification.
