User Acknowledgement Form definition
User Acknowledgement Form means the form signed by Civil Legal Services Subscriber’s Individual Users to confirm in writing that the Individual User has read and understands the requirements and restrictions in this Agreement (Exhibit A).

Examples of User Acknowledgement Form in a sentence
Each User shall sign a User Acknowledgement Form, either one developed by the Participating Agency or the Data Integration Hub or the one attached and incorporated herein as Exhibit B.
The access, use or dissemination of Court Data Services or Court Records (including Court Documents) by a Government Subscriber’s Individual User that has not completed a User Acknowledgement Form as required in Clause 7.2.1 is a violation of this Agreement.
