Unit record system definition

Unit record system means a system that provides a single record which is a composite of all data gathered during treatment of a given client, whether on an inpatient, outpatient, or emergency basis. The client is assigned a number on their first admission to the center which then is their [sic.] for all subsequent admissions and treatment at the facility. The entire client record is thus in one (1) folder under one (1) center number. Active records are maintained on the unit where the client is being treated.   • Record system means any group of records under the control of the division from which a record may be retrieved by a personal identifier such as the name of an individual, number, symbol, or other unique retriever assigned to an individual.

  • Designated Record Set shall have the same meaning as the term “designated record set” in 45 C.F.R. § 164.501.

  • Share Distribution Record Date means the close of business on the day which is four Business Days after the date of the Arris Meeting or such other date as agreed to by Arris and CLI and QMI and AHI, which date establishes the Arris Shareholders who will be entitled to receive CLI and QMI and AHI Shares pursuant to this Plan of Arrangement;
