UB-92 definition

UB-92 means the health care financing administration form UB-92 or successor forms.
UB-92 means the billing form identified by the Federal Health Care Financing Administration as HCFA Form 1450, as recommended by the National Uniform Billing Committee and adopted by the Kentucky Uniform Billing Committee for use by hospitals and other providers in billing for hospitalizations.
