Transitional Care definition

Transitional Care means assistance for a Member when entering and leaving an Acute care facility or a long term care setting.
Transitional Care means treatment of mental or nervous disorders, or substance abuse disorders in a less restrictive manner than inpatient hospital services, but in a more intensive manner than are outpatient services (includes but is not limited to day hospitalization).

Examples of Transitional Care in a sentence

  • The Care Coordinator is accountable for providing care coordination activities, which include ensuring appropriate referrals and timely two-way transmission of useful patient information; obtaining reliable and timely information about services other than those provided by the PCP; participating in the Enrollee’s Comprehensive Assessment, if any; and supporting safe transitions in care for Enrollees moving between settings in accordance with the Contractor’s Transitional Care Management program.

  • Transitional Care Unit Services addressing the needs of children and adolescents, under age 19, in the custody of the Department of Children and Families (DCF), who need group care or ▇▇▇▇▇▇ care and no longer meet the clinical criteria for continued stay at an acute level of care.

  • The residential aged care and community services include a residential aged care facility, District Nursing, Social Support Group, Bed based and Community based Transitional Care Program.

  • Transitional Care Management – the evaluation of an Enrollee’s medical care needs and coordination of any other support services in order to arrange for safe and appropriate care after discharge from one level of care to another level of care, including referral to appropriate services, as described in Section 2.5.C.2.

  • The Contractor shall develop, implement, and maintain protocols with each Affiliated Hospital that support the coordination of Enrollees’ care, including transitions of care, as part of the Contractor’s Transitional Care Management program as described in Section 2.3.F.

  • The Contractor shall have a Transitional Care Management program.

  • Transitional Services means services involving technical or specialized knowledge required to complete a project or to provide temporary consulting services to the City.
