Transition Home definition

Transition Home means a temporary housing accommodation and supportive services to homeless persons to facilitate movement to independent living, such as a temporary residential shelter.

  • The CONTRACTOR shall perform the CNA in-person at the Member’s primary residence unless the Member is homeless or in a Transition Home; the Member is part of the justice-involved population preparing for release; or the Member is a newborn in an inpatient setting.

  • Transition Date means the date on which this contract comes into effect for all purposes. Schedule 1 (Contract Particulars)
