Evaluation: CONTRACTOR is expected to adhere to the methodology and data collection outlined by the State Department of Health Care Services (DHCS) for the MHSA Full Service Partnerships, to evaluate its programs, including but not limited to copies of data collection tools, Partnership Assessment form, Key Tracking Event and the Quarterly assessment form.
