Teacher understands and agrees that failure to comply with this Contract and its requirements and deadlines may result in cancellation of this Contract for Teaching Services.

The Provider and individuals performing Educational Teaching Services through Provider shall have access to District records (including, but not limited to student and personnel records) and information contained therein only to the extent necessary for performance of the Educational Teaching Services.
As the Contractor's sole compensation for providing Teaching Services to Students and the rights granted under this Agreement, the Company shall pay the Contractor an amount equal to $_______ per hour (the "Fees"), payable by the tenth day of each month.
