Support Facilitation Service definition
Support Facilitation Service means services that empower Participants to define and direct their own personal assistance needs and services. The Support Facilitation Service is managed by a Supports Facilitator. The Support Facilitator guides and supports, rather than directs and manages, the Participant through the service planning and delivery process. The Supports Facilitator counsels, facilitates and assists in the development of the Individualized Service Plan and in facilitating the Participant in securing and employing their staff. The Support Facilitation Service also includes the services provided by fiscal intermediary.
