All programs will forward the fieldwork requirements to the School Practicum Site for their respective Student Practicum Candidates.
Preceptor: I have read and understand the duties and responsibilities of the Student in the MPH Student Practicum Agreement and I concur with the Student’s duties/assignments and learning objectives.
By signing below, I agree to execute my duties and meet my responsibilities as the Preceptor that are set forth above in order to help the Student satisfy the learning objectives and other requirements of the MPH Student Practicum Program.
Criminal Records Check All students seeking to complete placements within the NWT HSS System must provide a satisfactory Criminal Records Check, including a vulnerable sector check, to their educational institution (school), that has been completed within the past three (3) months, prior to commencing a Student Practicum Placement.
The Regional Managers will review the Student Practicum Placement Request Forms to determine if there is capacity to place the student within their region and will respond to Student or Instructor within 2 weeks of the virtual introduction.
The information may be provided to the Strategic Learning and Talent Development (SLTD) Unit at ▇▇▇▇▇▇▇▇▇▇▇@▇▇▇.▇▇.▇▇ The SLTD Unit within the Northwest Territories Health and Social Services Authority is responsible to lead the process with the Education Institute on behalf of the HSS system, to secure or renew all Practicum Affiliation and Student Practicum Placement Agreements.
Placement Commencement Once all of the pre-placement requirements are met (current Practicum Affiliation Agreement, proof of vaccination status and TB screening and an acceptable Criminal or Police Records Check with vulnerable sector check included), the Student Practicum Agreement, the Individual Practicum Supervisor Agreement Forms are received by the SLTD Unit and the Student has secured their required Out of Territory/Province Medical Travel Insurance, the Student may begin their practicum placement.
The process will be initiated by the submission of a Student Practicum Placement Request Form with supporting documents (resume, cover letter and proof of enrollment) to the Strategic Learning & Talent Development Unit at the NTHSSA.
The student assigned to a practicum experience at the Practicum Site shall be required to sign a Student Practicum Experience Agreement (attached to this Agreement and made part of it) before the student begins the practicum experience at the Practicum Site.
Practice Student, Practicum Student, Practice Teacher, Student Teacher, University Student, or Candidate, as used herein and elsewhere in this Agreement refers to a candidate enrolled in a credential program at the University in one of the Covered Categories, which leads to a Preliminary Credential, or to a Service Credential or who are otherwise enrolled in an undergraduate program at the University designed to prepare future teachers.
