• Based on the behavioral crisis and clinical need, grantee will coordinate transportation of individuals experiencing behavioral health crises, as defined in 26 Texas Administrative Code Chapter 301, Subchapter G (Mental Health Community Services Standards) to the appropriate service location where the individual can receive crisis stabilization services.

• ZEEL shall provide requisite number of Integrated Receivers and Decoders (IRDs) subject to payment of stipulated interest free security deposit per IRD as per the conditions laid down in the Telecommunication (Broadcasting and Cable) Services Standards of Quality of Service and Consumer Protection (Addressable Systems) Regulations, 2017 dated 3rd March, 2017.

• The Customer Services Standards in effect as of the Effective Date of this Franchise are attached as Exhibit A.

• Exemption certificates for Federal excise taxes will be furnished upon request by the Office of General Services, Standards and Purchase.

• The performance measures are based on the number of individuals with disabilities served (i.e., the number of individuals who have been determined eligible and have an independent living plan during the current state fiscal year), the number of individual goals met case closures and other standards as set forth within the HHSC Independent Living Services Standards for Providers.

• As at the date of the most recent variation of this Agreement the Approved Service Standards are the Health and Disability Services Standards NZS 8134:2008, comprising: • Health and Disability Services (General) Standard, NZS 8134.0:2008; • Health and Disability Services (Core) Standards, NZS 8134.1:2008; • Health and Disability Services (Restraint Minimisation and Safe Practice) Standards, NZS 8134.2:2008; • Health and Disability Services (Infection Prevention and Control) Standards, NZS 8134.3:2008.

• Publisher also may require certain Web Site modification services from YPS, which shall be provided by YPS in accordance with the Modification Services Standards set forth in the Web Site Specifications.
