Service Form definition

Service Form means any registration form signed by any Customer for the subscription of Services or modification to Services, any service form issued by the Customer to HKBN for the delivery of Services or modification to Services, any quotation or agreement signed by the Customer and returned to HKBN.

  • Income Certification means a Tenant Income Certification and a Tenant Income Certification Questionnaire in the form attached as Exhibit B hereto or in such other comparable form as may be provided by the Issuer to the Owner, or as otherwise approved by the Issuer.
