Sentinel Event Reporting definition

Sentinel Event Reporting means : For the purposes of reporting, a sentinel event is an event included in the list of Serious Reportable Events in Healthcare as identified by the National Quality Forum (NQF) and an event, identified as “inclusion,” that is a healthcare-associated infection as defined by the Agency for Healthcare Research and Quality. Additional reportable events may be identified by the State Board of Health

Related to Sentinel Event Reporting

  • Sentinel event means an unexpected occurrence involving death or serious physical or psychological injury or the risk thereof, including, without limitation, any process variation for which a recurrence would carry a significant chance of serious adverse outcome. The term includes loss of limb or function.

  • Day reporting means a program of enhanced supervision

  • Project Management Report means each report prepared in accordance with Section 4.02 of this Agreement;

  • Rapid(ly) report(ing) means within 72 hours of discovery of any cyber incident.

  • Root Cause Analysis Report means a report addressing a problem or non-conformance, in order to get to the ‘root cause’ of the problem, which thereby assists in correcting or eliminating the cause, and prevent the problem from recurring.
