Primary medical provider definition

Primary medical provider means a personal provider trained to provide first contact and continuous and comprehensive care to a member, chosen by a member or to whom a member is assigned under the Iowa health and wellness plan as the member’s primary medical provider.
Primary medical provider means the personal provider as defined in section 135.157 chosen by a member or to whom a member is assigned under the Iowa health and wellness plan.
Primary medical provider or "PMP" – a Participating Provider in Your Delivery System practicing and duly licensed as a Physician in family practice, internal medicine, gynecology, obstetrics, or pediatrics, and who has agreed to assume primary responsibility for Your medical care under this Contract.
