The benefits in force are those in place at the time of ratification and as described in RWAM Green Shield Canada Canadian Mental Health Association – Oxford Branch Billing Division No. 40069 Group 17029-1-A Outline of Benefits Booklet as attached in Appendix C this Agreement.
The carriers of Benefit Plans may be changed during the term of this Agreement provided that the benefits are comparable to the benefits currently provided by RWAM Insurance Administrators Inc.
Coverage is in accordance with the terms of the policies with the RWAM Insurance Administrators Inc.
When the services are extended for more than 30 days, prior approval must be obtained from RWAM on a monthly basis.
The Participant agrees to enroll in the NPAA Health Benefits Program and abide by all terms and conditions outlined in this Agreement, the Health Benefits Policy, attached hereto as Schedule “A” and as amended from time to time by the NPAA at its sole unfettered discretion, and the RWAM Group Benefits Master Application (the “RWAM Agreement”), attached hereto as Schedule “B”.
RWAM will take reasonable steps to ensure the integrity and functioning of the System.
The User shall designate (by completing Schedule A) no more than two individuals (either employees of the User, or if agreed to by RWAM in advance an employee of a third party contracted by the User) to be granted access to the System (hereinafter, a “Plan Administrator” or “PA”).
Schedule A (Plan Administrator Appointment) and Schedule B (RWAM Privacy Statement), and any subsequent amendments or substitutions to either Schedule, are hereby incorporated by reference into this Agreement.
Approval of the treatment plan must be obtained from RWAM prior to commencement of treatment (except for emergency treatment required to alleviate pain).
An “Authorization Form for RN Services” must be completed by the attending physician and submitted to RWAM.
