Root Cause Failure Analysis or “RCFA” shall mean an assessment conducted pursuant to Section IV.D (Root Cause Failure Analysis for Bypass Venting Incidents) of this Consent Decree to determine the primary cause and any contributing cause of a Bypass
Root Cause Failure Analysis means analysis based on a case-by-case investigation that seeks to discover the event that caused a Product failure, which, once removed, will prevent a like event from occurring again.
