Responsible provider definition

Responsible provider means a health care provider who is primarily responsible for patient care in a health care facility or provider's office.
Responsible provider means the physician or other licensed health care provider named on the specimen collection form, or the newborn’s primary health care provider as identified to the testing laboratory.
Responsible provider means an applicant that has the capability in all respects to perform fully the contract requirements.
