Requesting Provider, definition

Requesting Provider, means the provider requesting aid, and
Requesting Provider, means an individual healthcare professional who is making a request on their own behalf; it does not include a health system, a health group administrator, or a practice group. For purposes of clarity, “administrator” shall not include a provider’s secretary, office administrator or other administrative staff who may typically prepare or execute paperwork for an individual provider.

Examples of Requesting Provider, in a sentence

  • If the reason for the delay is because the Covered Person’s review request lacked sufficient information to complete our review of the Covered Person’s request, the Covered Person, the Covered Person’s Designated Representative and Requesting Provider will have 45 days from receiving our notice of delay to provide the necessary information.

  • The Requesting Provider option is presented if the service offered by the provider is Diagnostic or Pathology.

  • We will notify the Covered Person, the Covered Person’s Designated Representative and the Requesting Provider of our decision within not more than 10 days after notifying the Covered Person, the Covered Person’s Designated Representative and Requesting Provider of the reason for our delay or of our receipt of the necessary information or of the end of the period afforded to the Covered Person to supply the necessary information, as applicable.

  • The Referring and Requesting Provider Setup (listed on the terminal as “REF/REQ Provider”) option allows you to add, edit, delete and review current Referring and Requesting Providers.

  • The default is for no Requesting Provider, with an override code depending on the Service type.

  • The Form shall provide the option for a Requesting Provider to request and consent that Healthgrades to remove from the Requesting Provider’s Profile page any and all references to other individual healthcare providers; specifically, the removal of providers illustrated, through example, as depicted in Exhibit D.

  • The Substituted Service option is presented if the service provided is diagnostic and a Requesting Provider was supplied.

  • A ‘Y’ indicates that Requesting Provider details have been indicated.

  • A Requesting Provider must submit the Form; it is not sufficient for a practice group, a health group administrator, or a health system to submit Forms for a Requesting Provider, multiple Requesting Providers, or an entire group or practice.

  • Healthgrades may require, as a condition of removing content, that the Requesting Provider “claim” their profile.

Related to Requesting Provider,

  • Hosting Provider means any Permitted Subcontractor that is providing any or all of the Hosted Services under this Contract.

  • Participating Provider means an Administrator Hospital or Professional Provider which has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to participants in the Participating Provider Option program or an Administrator facility which has been designated by the Claim Administrator as a Participating Provider.

  • Training provider means an organization meeting the eligibility conditions as mentioned in Data Sheet and selected in accordance with the criteria set forth for the purpose.

  • Nonparticipating provider means a person, health care provider, practitioner, facility, or entity, acting within their scope of practice, that does not have a written contract to participate in a managed health care system's provider net- work, but provides health care services to enrollees of pro- grams authorized under this chapter or other applicable law whose health care services are provided by the managed health care system.

  • New Provider means any replacement service provider or providers engaged to provide the Services (or part thereof) or substantially similar services or the Authority itself where the Services or substantially similar services or part thereof continue to be provided by the Authority after partial termination, termination or expiry of this Contract;
