Rendering Provider ID definition

Rendering Provider ID means an identification number, assigned by OHA, used to represent the OHP Participating Provider delivering Covered Services on behalf of the OHP.
Rendering Provider ID means an identification number, assigned by DMAP, used to represent the OHP Participating Provider delivering Covered Services on behalf of the OHP.

Related to Rendering Provider ID

  • Hosting Provider means any Permitted Subcontractor that is providing any or all of the Hosted Services under this Contract.

  • Provider Number means an identifying number issued to each homecare worker who is enrolled as a provider through the Department.

  • Local Service Provider (LSP) means the LEC that provides retail local Exchange Service to an End User. The LSP may or may not provide any physical network components to support the provision of that End User’s service.

  • Training provider means an organization meeting the eligibility conditions as mentioned in Data Sheet and selected in accordance with the criteria set forth for the purpose.

  • Provider agreement means the signed, written, contractual agreement between the department and the provider of services or goods.
