Publicity Release. In connection with my participation in the Event, I give permission for Releasees to use my name, photograph, likeness, and voice in any manner and in all media now or hereafter known, in perpetuity throughout the world, without compensation of any kind.

Publicity Release. We hereby irrevocably authorize ▇▇▇▇▇, its successors and assigns, and those acting with its permission and upon its authority, to photograph videotape or film my child (or talent by my child) for advertising, publicity or any other lawful purpose for ▇▇▇▇▇. • The selected finalist must also verify eligibility within 15 calendar days after notification WSB and sign and return an Affidavit of Eligibility, a Release of Liability, and a Publicity Release form (collectively the “Verifications Forms”).

• Please initial below to indicate consent to the Publicity Release as described above: I consent to the above Publicity Release authorizing my name, photograph, and other identifying information to be used for publicity, promotional or other purposes by the Corporation for National and Community Service, the Michigan Primary Care Association, the MPCA Community HealthCorps program, my Service Site, and/or the Michigan Community Service Commission.

• Your organization, as recipient of the grant made in connection with America’s Farmers Grow Communities (the “Project”) sponsored by the Monsanto Fund, is required to agree to the Publicity Release provided on the next page.

• Each Winner may be required to sign, at the request of and in the sole discretion of the Houston Rockets and/or Sponsor, a Release, an Affidavit of Eligibility, a Publicity Release, and a Prize Acceptance Form in order to receive the Prize.

• The provisions of Articles 18 - Inventions and Improvements, 21 - Compliance with Law, 22 - Publicity Release, 23 - Confidential Information, 24 - Patents and Information and 25 - Warranty and Liability shall survive the termination of this Master Agreement and any Development Agreement formed hereunder.

• Epic Brands requires each participant of your team or teams, and if such participant is under the age of 18 their parent or guardian, to sign a Lifetime Waiver of Liability, Indemnification, Assumption of the Risk, Authorization To Contact Emergency Medical Assistance and Publicity Release Form (“Release Form”), found at the link set forth below in this paragraph.
