Provider Choice definition

Provider Choice means Provider Choice the business with ACN ▇▇▇ ▇▇▇ ▇▇▇;
Provider Choice means Provider Choice Pty Ltd (ACN ▇▇▇ ▇▇▇ ▇▇▇); and “you” “your” and “Member” means a person that has signed up to be a Member of Provider Choice or a person using the Provider Choice Platform and eMarketplace.

  • GHP manages behavioral health services for participants of Provider Choice and PPO.

  • HBF applies the Medicare assessment rules to calculate benefits payable under the Full Cover Schedule, Provider Choice Schedule or the Specialist Anaesthetist Schedule (HBF Schedules).

  • Magellan manages behavioral health services for participants of the Provider Choice and PPO plans.

  • Not allow any of the LIDDA’s staff from its provider operations to initiate contact with the individual or LAR prior to the completion of the Documentation of Provider Choice form.

  • To the extent permitted by law, you agree that we (Provider Choice) are not responsible or liable to you in relation to any of the following: • Information posted on the Provider Choice Platform by a Provider about the Provider or the Provider’s goods and services; • Transactions you enter into with a Provider and the Provider’s goods and services; • Your use of or reliance on the Provider Choice Platform; and • Any delay, error or failure in respect of the Provider Choice Platform.

  • All third party trademarks (including logos and icons) referenced by us and Providers on the Provider Choice Platform remain the property of their respective owners.

  • All salaries will be raised by 1.3% on 1 October 2007, by 1.0% on 1 October 2008, by 1.0% on 1 October 2009 and by 2.4% on 1 July 2010.

  • Nothing in this section limits any rights you have under the rules that apply to any payment scheme through which you pay for goods or services purchased through the Provider Choice Platform.
