Primary Healthcare means healthcare provided by a General Practitioner, practice nurse or similar professional who acts as the initial principal point of consultation and who may co-ordinate any other specialist(s) a Refugee might need.

  • Treatment case files must include, at minimum, the Primary Care Physician’s name and address, a signed waiver release of information for purposes of coordination, a statement that the Covered Person has refused to sign this waiver, or a statement that the Covered Person does not have a Primary Healthcare Provider.
