Unless otherwise indicated, Eligible Expenses are paid at 90% of Preferred Allowance (PA) for in-network (PPO) providers and 70% of Usual, Reasonable and Customary (URC) for out-of-network (non-PPO) providers for Medically Necessary services and supplies, and include the following, subject to the limitations and exclusions indicated.
